Health Certificate Driving License

What is it?

Are you healthy enough to get a driver's license or renew your license? CBR assesses this with the Health Declaration. You fill this out yourself. This way you know for sure that you are safe on the road.

There are 3 types of health certificates:

  • Health Certificate Driving License
  • Health Statement Large Driving License
  • Health Statement 75+

From January 1, 2022, it will no longer be possible to collect a paper health certificate for Driver's License and Large Driver's License from the municipality.

You can still pick up the health certificate 75+ at the municipality, but you must make an appointment. You can do this online or by calling 14 045.

How does it work?

There are two ways to order your health certificate through the CBR.

1. With DigiD

If you need a Health Statements for the Driving License or Large Driving License, you can fill out the Health Statements digitally via DigiD through My CBR(http://mijn.cbr.nl). Here you purchase your Health Statement online and complete it immediately. The advantage of submitting digitally is that it is faster than the paper application. If you submit the health certificate digitally, you can view the status of the application at any time through My CBR. Submitting the Health Declaration digitally via DigiD speeds up the process.

2. Without DigiD

If you do not have the ability to complete the Health Declaration digitally via DigiD, you can order a paper Health Declaration. This costs an additional 11.35 euros due to the extra manual work.

The ordering options for paper are:

  • Health declaration for all driving licenses: through the CBR web shop. This can be done without DigiD and is sent within 5 working days. Pay via iDeal.
  • Health declaration for all driving licenses: at the CBR exam locations (Amsterdam, Rijswijk, Eindhoven and Arnhem). Payment only possible by pin.
  • Health declaration for large driver's license: through a number of Occupational Health Services and employers.
  • Health statement for driver's license special needs, BTS and people in rehabilitation pathway: through rehabilitation centers.for driver's license special needs, BTS and people in rehabilitation pathway: through rehabilitation centers.
