Child care, apply for contribution with socio-medical indication

What is it?

Parents with physical, psychological or social problems can sometimes (temporarily) no longer cope with the upbringing and care of their child(ren). Child care can then be a solution. The scheme Social Medical Indication (SMI) childcare allows parents to receive compensation for the costs of childcare.

How does it work?

Requirements

  • You or your partner has serious physical, psychological and/or social problems or
  • Child care is necessary in the interest of proper and healthy child development.
  • You do not receive compensation for child care through the municipality, the UWV or the Tax Office
  • You cannot use other facilities such as preschool, medical daycare or other alternatives.
  • A specialist or involved social worker must indicate in writing that care is needed for your child.

Level of compensation

For the amount of the contribution, the municipality uses the trial calculation on Toeslagen.nl of the Tax Office. The amount resulting from this trial calculation is increased by the municipality by 4.5% of the costs for the first child. For each subsequent child, this increase is 3.5% of the costs.

What to do.

To apply for the contribution, you can request the application form by phone by calling the general number of the municipality, tel: 14 045. Choose option 4 number 1.  

What should I bring?

  • valid proof of identity: passport, European identity document or residence document (no driving license!).
  • A letter from a medical specialist or social worker stating why child care through a Social Medical Indication is needed
  • A contract or quote from the child care facility that includes the number of hours of child care per child, the cost per hour and the start date of care
  • An authorization to request information from the child care provider regarding the number of hours attended at the child care center/host parent and the justification of the allowance
  • A statement of your income for the last 3 months.
  • As complete an overview of your assets and debts as possible.
  • Statements of all your bank and savings accounts for the last 2 months. Also of your minor children.

Do you have benefits under the Participation Act, IOAW, IOAZ or Bbz 2004? What is already known, you do not need to submit again.

How long does it take?

After making the application, you will receive written notification from the municipality within 8 weeks.
