Name change surname

What is it?

  • If you want to change your last name or that of your child, first check the Justis brochure Name Change to see if this is possible. Justis is the screening authority of the Ministry of Justice and Security.
  • If your situation is in this brochure, you can apply for a name change at Justis.

What to do.

  • Download and complete the application form.
  • Check the Name Change brochure to see which code belongs to your situation. Fill in this code on the application form. Fill in this code on the application form.
  • Add the requested documents to the form and send everything to Justis.

You will automatically be notified if your last name is changed. You will then receive an official extract from the Royal Decree.

Customizing documents

  • If your last name has officially changed, then it must also be changed in your identity document. Therefore, apply for a new passport, identity card and/or driver's license.
  • Health also make sure you inform all authorities about your changed name. You do not need to inform the municipality separately. It already automatically receives an extract of the Royal Decree.

How long does it take?

You will be notified through Justis within 20 weeks whether your last name will be changed. This may take longer if your application is not complete.

Objecting

If you disagree with the decision, you can file an objection with Justis within 6 weeks.
